Test request form and sample transport
In compliance with the ISO 15189 standard, the request must contain the following information:
- Identity of the patient
- Identity of the prescribing physician
- Clinical information (if relevant)
- Tests prescribed
- Nature of the sample
- Identity of the individual collecting the sample
- Date and time of the sample
- Sample reference
All samples must be labelled unambiguously
The samples must be packaged in a suitable manner to protect the haulier from any risk of contamination. During the transportation, extreme temperatures and sudden temperature changes must be avoided. The tests directory details the specific requests relative to transport timing and temperature.
If the above is not followed, the analysis might not be performed at all, or results will be subject to confirmation.
